Infertility

Infertility is a medical term used to describe the inability of a person, or a couple, to conceive naturally. This condition can affect anyone, regardless of gender, sex or age. According to research, around 15% of couples trying to conceive will fail to do so while following traditional methods. Infertility problems affect women and men equally.

If the patient is under 35, the doctor will diagnose fertility issues after 12 months of regular, unprotected sex. As the patient ages, their doctor may diagnose fertility problems earlier. Patients over the age of 35 are diagnosed after 6 months of trying to conceive.

Types of infertility

Infertility can be categorized into three main groups, depending on the reason or the characteristics of the patient. The categories are:

  • Primary infertility: Patients suffering from primary infertility are not able to conceive after one year, or six months of trials. They have never achieved pregnancy.
  • Secondary infertility: Patients suffering from secondary infertility cannot conceive, after at least one successful pregnancy.
  • Unexplained infertility: Regardless of previous pregnancy success, patients with unexplained infertility cannot achieve pregnancy for an unknown reason.

Infertility causes

According to studies, infertility causes affect both parties of a couple equally. There are many factors that play key roles in the inability to achieve pregnancy. Age is one of the most important, with women gradually losing their fertility capabilities after the age of 35. Men will see their ability to reproduce deteriorate after the age of 50.

Factors such as eating disorders, radiation therapy, substance abuse, obesity, and sexually transmitted infections will also affect the patient’s ability to reproduce. Depending on the sex, fertility issues can be caused by a multitude of reasons.

Women

Ovulation problems are usually the main cause of infertility in women. During ovulation, an egg is released by the ovaries in order to be fertilized by the sperm. Factors that can disrupt ovulation consist of the following:

  • Polycystic Ovarian Syndrome (PCOS)
  • Thyroid disease
  • Genetic and chromosomal disorders
  • Absent menstrual cycles

Other conditions that can affect a woman’s ability to reproduce include:

  • Endometriosis
  • Fibroids or polyps
  • Primary ovarian insufficiency
  • Autoimmune disorders like lupus or sickle cell anaemia

 

Men

Sperm disorders, such as limited motility and low sperm count, are common causes of infertility in men. Other factors may include:

  • Varicocele (enlarged scrotum veins)
  • Injuries in the scrotum or testicles
  • Low testosterone
  • Ejaculation dysfunctions, such as anejaculation or premature ejaculation
  • Undescended testicles
  • Anabolic steroid abuse
  • Exposure of the testicles to high temperatures

Does infertility cause symptoms?

The main symptom of fertility issues is the failure to conceive after either six months, or one year of trials. Patients may not experience other symptoms, or show symptoms based on the condition that causes it. Physical symptoms usually include the following:

  • Irregular or absent menstrual cycles.
  • Vaginal bleeding between cycles.
  • Abdominal pain.
  • Disorders regarding the penis or ejaculation.

Diagnosis and Tests

Diagnostic exams for infertility involve a series of tests, according to the patient’s sex. Female infertility is often diagnosed using pelvic exams, blood tests and transvaginal ultrasounds. More complex examinations, such as a hysteroscopy or a sonohysterogram may be necessary if the cause is not easily identifiable.

Males struggling with fertility issues will undergo tests in order to determine their sperm quality. These include a semen analysis, blood tests and, if necessary, a scrotal ultrasound.
